VIN approval form (CA01) (previously known as VCUEF 01) is used apply to affix or re-affix a VIN to a New Zealand registered vehicle.

Each time you apply you need to pay for a unique PDF download of the application form from Blue Star. You can then complete the application.

To apply:

1. Order a VIN approval form in the Blue Star portal and pay by credit or debit card.
2. Blue Star will email you a PDF which you can print, fill out and scan (please keep an electronic copy).
3. Send the completed form to mvapprovals@nzta.govt.nz for processing.
4. We'll use the unique serial number on the form to confirm you've paid.

You can purchase multiple PDFs in a single transaction. Each PDF costs $39.70.
