Fit and proper person disclosure and consent for inspecting organisations and specialist certifiers (FP01) 

This form is used to support a new application for appointment as a:

  • warrant of fitness inspecting organisation (IO)
  • certificate of fitness IO
  • new light entry vehicle inspector (VI) (also known as a new light entry certifier)
  • heavy vehicle specialist certification: local manufacturer IO.

Inspecting organisation application: warrant of fitness, certificate of fitness and additional sites (IOAPP) 

This form is used when applying for an NZTA warrant of fitness (WoF) or certificate of fitness (CoF) appointment as an inspecting organisation (IO), or when an existing IO is applying to add another inspecting site to their appointment.

Application to change inspection groups (IOCH)

Use this form if you're an appointed WoF and/or CoF inspecting organisation applying to add vehicle inspection types within your current WoF and/or CoF appointment.

This form cannot be used to add a new vehicle inspection appointment.

This form replaces the WOFCH and COFCH forms.

Site relocation application (IO relocate)

This form replaced the WoF/AF relocation application and CoF relocation application forms.

Appointed inspecting organisation can use this form to apply to relocate an inspecting site. An IO RELOCATE form must be completed for each additional site.

Surrender an IO, VI, or specialist certifier appointment (VOLSUR)

Use this form if you want to surrender your Inspecting Organisation (IO), Vehicle Inspector (VI), or specialist certifier appointment. If
you are surrendering an IO appointment, you must have authority to act on the organisation’s behalf.

This form is used when applying for reappointment as a NZ Transport Agency Waka Kotahi (NZTA) warrant of fitness (WoF) or certificate of fitness (CoF) inspecting organisation (IO).
