IOCH: Application to change inspection groups

This form replaced the WOFCH and COFCH forms.

Use this form if you're an appointed WoF and/or CoF inspecting organisation applying to add vehicle inspection types within your current WoF and/or CoF appointment.

This form cannot be used to add a new vehicle inspection appointment.
